CCI’s onsite Health & Wellness Service Center provides quality primary and urgent care to adults and children. Backed by professionals who are committed to your good health that have been serving the community since 1972. CCI’s expert, dedicated and certified staff are well-trained, friendly, and speak 22 languages among us. If you are looking for a patient-centered medical home, CCI can offer you a full array of primary care services to get well and stay healthy.
Medical Care
Community Clinic maintains a strong commitment to comprehensive, affordable, quality primary care for children and adults. Board Certified Physicians, Dentists, Nurse Practitioners, Physician Assistants, Hygienists, Nurses, Behavioral Health Therapists, Nutritionists, Registered Dieticians, Dental and Medical Assistants, and trained support staff care for our patients and their families. CCI provides health care for the “whole person” and offers additional services a patient may need in a primary care setting such as mental health counseling, preventative dental services, nutrition assessments and counseling, diabetes and chronic condition management, and family planning. Medical care services available at all CCI health center locations include:
- Well-care for children and adults
- Acute and chronic care including gynecology and preventive services
- Routine immunizations
- Family planning health care and counseling
- Diabetes education and nutrition counseling in English and Spanish
- Behavioral health counseling for children, adults and families
- Referrals to medical specialists and follow-up
- Primary care laboratory services
Dental Services
A big part of good oral health is making regular appointments to see a dentist for cleaning, exams and x-rays. CCI dentists and the dental staff welcome CCI patients to make appointments for dental care. CCI Dental Services are available to all ages. They encourage their Medical Service patients participants to take advantage of this added service. Family members of medical services patients are encouraged to make an appointment and come in for a dental visit as well.
